WARNING: Since I've moved this server to a NetBSD/mips
machine rather than a NetBSD/alpha machine, this functionality is
currently broken. I will be fixing it at some point, soon, though.
First, a quick description of the fields.
Broken Firmware can be set to either, Yes or No.
Basically, if you need this form try both. I have a
Multia and it works only with the "No" option.
Ether address in %x:%x:%x:%x:%x:%x form.
Anyway, you'll find that a binary
of type application/octet-stream is returned to you if
all goes well.
